This easy guide talks about Outreach Support in simple words and pictures

Outreach support is background support to help you keep your tenancy agreement.
A tenancy agreement is a contract between you (the tenant) and the owner of the property (your landlord).

We will work with you to write a plan so that we know what is important to you and how you want to be supported.

We can help with understanding letters, emails, bills and forms

We can help with making health appointments

We can help with finding technology to help you do things for yourself

We can also help you find ways to get along with your neighbours and show you where to find more services or support that might help you live independently.

Outreach support does not help with taking your medicine or your personal care. You will need to arrange other support if you need help with this.

If you need more than 30 hours support each week, you may find Independent Living Support is a better option for you.

To use our services, you need to have money ready to pay for your care, this is called funding.

You can talk to your social worker to help you make a plan for your support. They will help look at how you can pay for your support.

You can pay for your care with your own money, or use direct payments. Or your Social Worker can arrange payments.

If we can help you, we will talk with you and your family. Together, we will make a plan for your support.
It will explain how and when you have support from us.

We will help you get used to your new support. This might mean visits, overnight stays, or meetings with your support team or people you will live with.

When you are ready, you will start getting support from us. We will check how things are going after 6 weeks, 6 months, and every year.
