Community Lives - Day Activities
Our day support has fun activities. People can learn new things, make friends, and look after their well-being.

We will work with you to write a plan so that we know what is important to you and how you want to be supported.

We have lots of choices like:
- volunteering
- fun activities
- learning new skills

We know everyone is different. That’s why we offer options for people to pick the activities they like best.

Our team can help you with taking your medicine or your personal care while you are at your activities.

We know friendships and being part of your community is important. Our activities can help you meet new people and make new friends.

To use our services, you need to have money ready to pay for your care, this is called funding.

You can talk to your social worker to help you make a plan for your support. They will help look at how you can pay for your support.

You can pay for your care with your own money, or use direct payments. Or your Social Worker can arrange payments.

When you have the funding agreed with your social worker, you can call us to talk about what support you want. Our phone number is
01189 778 600.

If we can help you, we will talk with you and your family. Together, we will make a plan for your support.
It will explain how and when you have support from us.

We will help you get used to your new support. This might mean visits, joining us for part of the day or meetings with your support team.

When you are ready, you will start getting support from us. We will check how things are going after 6 weeks, 6 months, and every year.
