Brokerage and Support

The Brokerage and Support Team work with adults who have eligible needs for adult social     care services. This includes people who have physical disabilities, learning disabilities, or are  elderly. We provide support to individuals with personal budgets. We help to plan the support according to your needs, implement your support plan, and review the needs and outcomes.

Cockayne Court

Extra care housing scheme: personal care and support delivered in your own home to suit your eligible social care needs. 24 flats and 19 bungalows, the scheme is manned by care staff 24 hours a day and daily activities are available for tenants to attend.

Domiciliary Care Service - including START and Long Term Support

The purpose of our Domiciliary Care Service is to provide effective, efficient and specialist personal care and support to vulnerable individuals within their own homes and within the community.

Earley Learning Disability Day Service @ the Crescent Centre

The Crescent Centre in Earley is well-established as a base for our activities. It’s near the Earley shopping centre and the library. We use Loddon Valley Leisure Centre for swimming, the Nirvana Spa and Pulse-8 for keep fit, and we also have links with Maiden Erlegh Park.

Foot Care Service

We recognise the importance of maintaining good foot care as a preventative measure for those who cannot carry out their own care and are not eligible for NHS podiatry care. Foot care is provided by Home Care Assistants who have received training from an NHS Podiatrist, and provides four aspects of basic foot care: toenail cutting, filing of toenails, bathing of feet and cream application to feet.

Horticultural Therapy @ Growing Places

The gardening and conservation section of the learning disability day service; providing services to adults with learning disabilities. Our ethos is that everybody who joins us, no matter what their skill, has a meaningful part to play within the team and opportunities to fulfil their potential.

Learning Disability Day Service

The Learning Disability Day Service provides support and activities for people with a wide range of needs. The service is supported by experienced staff. All parts of the service are community-based which enable us to provide a wide range of activities for the people we support.

The service operates through bases located around the Wokingham Borough:

  • Wokingham @ Acorn Community Centre (main base)
  • Woodley @ The Bulmershe Leisure Centre
  • Earley @ The Crescent Centre

Learning Disability Community Health Team

The team provides support to adults with learning disabilities. The core team of nursing and occupational therapy are based at the Old Forge in Wokingham town centre.  Other members of the extended team are speech and language therapy, physiotherapy, dietetics, psychology and psychiatry.

Leaving hospital: START (Short Term Assessment and Reablement Team)

This service is provided for a limited time period and enables people to maintain their independence in their own home. The specially trained team of START Assistants work between 7am and 10pm, 365 days a year. START will work with you to help you to regain your daily living skills as far as possible. During this time we will be able to establish any on going care needs you have.

Long Term Support

The team offers effective long-term care and person-centred support to individuals with dementia who wish to remain within their own homes. A person-centred approach is one that supports people with dementia to lead as fulfilling life as possible in all areas of daily living. Your needs may be met through a number of ways such as the assistance with personal care, the assistance to enable you to take part in social activities or help with shopping and other daily living tasks.

An evening and weekend service for people with a learning disability and people with a physical disability.

Pinehaven Day Service

Day service for older people. We aim to meet physical, social and emotional needs of clients through social activities and respect for individuals: needs of carers also addressed. The day service provides educational, recreational, health promoting and fun activities including cookery, gardening, discussion, music, relaxation with gentle exercises, quizzes, bingo, and crafts.

Sensory Needs Service

There are two teams within the Sensory Needs Service: The Visual Impairment Team and Deaf Services Team. These two teams provide specialist services for people with sensory needs. The teams offer advice, assessment, training and work with individuals.

Visual Impairment Team

Support for adults and children with visual impairments to help them live as independent and full a life as possible, whatever their needs or disability. The team is staffed by professional rehabilitation officers who can provide various services including equipment and home adaptations, day care, help from volunteer agencies and information and advice.

Deaf Services Team

The team provides services to Deaf, deafened, hard of hearing, and Deafblind adults. This is under an agreement between four Berkshire local authorities: Bracknell, Reading, Royal Borough of Windsor and Maidenhead, and Wokingham.

This also includes offering help to those who, in addition to their hearing impairment, may also have a physical disability, learning disability, mental health or who are elderly.

Suffolk Lodge Care Home

Suffolk Lodge provides accommodation and care for up to 40 people, who have care needs associated with old age and/or Dementia. The home is a purpose-built property located close to Wokingham town centre, with accommodation on the ground and first floor.

The Supported Employment Service

We provide a friendly and confidential employment support service for young people and adults with disabilities, mental health issues, HIV/Aids, substance misuse issues, long-term health conditions and their carers. We can help if you want a job but need support; are planning to return to work, want a new direction or new training, or need support when meeting with an employer.

Westmead Day Centre

Westmead Day Centre is a fully accessible service for adults with physical disabilities. We provide a range of services for the rehabilitation and respite care of people with physical disabilities that have a critical need (and also self-funders). Our services are flexible and the emphasis is on supporting and enabling you to develop your full potential and maintain your independence.

Wokingham Learning Disability Day Service @ Acorn Community Centre

Acorn Community Centre provides services and activities for the community, and people with learning disabilities to ensure they are part of the community. We offer a wide range of activities both at the Acorn centre and in the local community. Facilities include a sensory room which provides a calming and stimulating environment, an art room, a computer/internet suite, meeting rooms and a multipurpose hall for private or corporate functions. All of the Community Centre’s facilities are fully accessible.
