Optalis has two services which are community based:

  • Long Term Support team (LTS) – this is a service for Older Adults living with dementia. The main purpose of this service is to support our customers living within their own homes.
  • Short Term Assessment and Reablement Team (START) – this is a reablement service of up to six weeks (maximum) where people are supported with regaining confidence and independence in their own homes, following a hospital discharge, illness or accident. This team also works as an assessment service.

Both of these services cover the Wokingham Borough

We are looking for support workers to promote our values and to support our customers to live happy and content lives. You will be assisting with personal care, medications, nutrition needs and domestic tasks. Skills can be taught in the care industry, so experience is not essential. These roles are community based and you must have a full driving license and daily use of a vehicle.

Job Title: Senior Support Worker - START

Post Description: Undertaking first visits for new START customers and carrying out the necessary paperwork to reflect: risk assessments (general, moving & handling and medication); agree reablement goals and objectives; agree weekly reablement tasks/objectives. Supervising field care staff; carrying out 1 to 1 observations, field spot checks, regular supervisions and planned appraisals. Anticipate each Senior Carer will supervise 5-6 care staff. Assist with on-call shift rota which operates 07:00 – 22:00 365 days a year.

As a Team Support Officer you will work alongside and directly support the work of a busy and respected Supported Employment Service, ensuring the continued delivery of high quality service.

This role will see you assisting the Service Manager with contracts and the day to day running of the service which will include: Providing reports, forecasting, maintaining and development of report systems. Be able to respond promptly to service needs and the ability to prioritise service demands. You’ll engage with a range of internal and external stakeholders and be the first point of contact for our clients, families and carers.

This role is interesting, challenging and rewarding. As a Team Support Officer you will be required to work in a fast paced environment, taking new ideas and challenges on board quickly to ensure a high quality service. With some understanding of health and social care, you will have previous work experience from which you can demonstrate the required skills for the role.

We are looking for energetic and enthusiastic support workers to work in our unique Independent Living Service (ILS) supporting people with Learning Disabilities who live in their own home. We have services in the Wokingham Borough, including Barkham, Winnersh and Ryeish Green. For this role you need to be a car driver with use of a vehicle, and some houses have a manual transmission ‘house vehicle’ which you may be required to drive. As ILS is a very diverse service, for the right candidate, we may consider a non-care driver for an exceptional candidate. You need to be creative especially in the use public transport in order to support our customers with their daily needs. The hours for this role can vary depending on the customers care packages, but tend to start from 7am-3pm or 3pm-10pm (full-time support workers will have to be available for at least one sleep in every 7-10days, depending on the service). You will receive a comprehensive induction and on-going training programme, as well as regular supervision to ensure you are fully equipped to support our customers. You will also have the support of a Registered Manager and large staff team including Deputy Managers and Senior Support Workers. Optalis is currently looking for an Interim Registered Manager for our START service (Short-Term Assessment and Reablement Team). The START service forms part of the Wokingham Integrated Social and Healthcare Team (WISH). The two important functions provided by START are:

  1. A reablement service of up to six weeks (maximum) where people are supported with regaining confidence and independence following hospital discharge, illness or accident.
  2. An assessment service whereby a person’s reablement potential and/or on-going care needs are reviewed.

The service is available seven days per week, 365 days per year and works on a shift pattern from 7am – 10pm. The primary purpose of this role is to be responsible for ensuring START is compliant with the range of applicable CQC standards and, as part of the Wokingham Integrated Social and Healthcare Team (WISH), help ensure the delivery of a person centred service to support the health and wellbeing of the population. This role is estimated to last for 6 months.

As a Broker in the Review Team you will be helping to support adults to review their support plans and completing simple re-assessments with guidance. You will co-ordinate review meetings and identify agreed actions and follow up as required.

We are looking for someone who will be able to manage a busy caseload to maintain turnover of work, and in return we offer regular supervision and support as necessary, as well as a comprehensive Induction period. As a Broker you would need IT skills including working with Word and Outlook, plus the ability to work with any other IT systems as required. You will need good written and verbal communication skills and an excellent telephone manner and the ability to engage with people who may have communication difficulties. This role includes some lone working and you will need to be able to travel independently.

The Brokerage and professional support team is a busy, dynamic team who work with older people, people with physical disabilities and those with a learning disability.  We are based at Shute End, Wokingham.  The successful applicant should be a qualified, experienced social worker, with some management experience. The role of the service manager is:

  • To be part of a strong management team and assist with the development and operational management of the service.
  • To take specific management responsibility for the Safeguarding Lead and staff development

Our Services provide support to adults with learning disabilities or physical disabilities to access day, evening and weekend activities.

We are looking for an enthusiastic and motivated support worker to join our team of dedicated staff.

You will need to be able to work independently, using your own initiative and on occasions, be able to manage challenging situations.  You should have particular skills in planning activities and motivating others.  Ideally, you would have one year’s experience of supporting vulnerable adults in the community but this is not essential.

You would need to be able to drive and be willing to use your own car if required for which we pay a generous mileage allowance, as well as be able to drive our own people carriers available for long distance journeys/group activities. Our ideal candidate would be willing to commit to working one day at the weekend as part of their hours – the day can be negotiated.

The purpose of this team is to avoid unnecessary admission to hospital and to keep our vulnerable adults safe in emergency circumstances such as their main career being taken to hospital and requiring support in their absence.
